Forms, forms, forms

So, if you haven’t seen on facebook, I went back to work on Tuesday.  The kids started Thursday, so I began doing my usual routine, tracking down kids on my IEP list and getting prepared for starting to create my schedule.  I wanted to share a few forms that I use for letting my teachers know about how to refer kids.  I rely heavily on my teachers to be my initial eyes and ears for future speech kiddos and it’s great to hear their insights about what they see in the classroom setting.

At the beginning of the year, I like to send a cover letter with a referral form to my teachers, so that they can let me know of any students who they may suspect need speech or language services.  It is always helpful to have the referral in written form because then I just slide it in my caseload binder as a reference of who I need to observe next when I have a free minute (like that is very often).  This system has helped me a lot because I can get an idea of what the teacher is observing along with an additional information they may have gathered.  I made my cover sheet with a summer/beach theme……if you haven’t been checking out my past posts, I am doing a summer theme in my room this year!

These days it is more and more important to keep accurate attendance logs because if we do not show we are fulfilling a student’s IEP, then we could be taken to litigation!  Here is a FREEBIE Attendance Forms by Natalie Snyder to use to keep track of who’s here or not here!

I also made this cute binder cover for my student log sheets where I keep my daily notes for each IEP student, their information sheet in case I need to call home and a contact form, so that I can document when I call a parent.  Just click on the picture to get that for your student binder!  Teach Speech 365 has lots of FREEBIE forms that you may like to use for your therapy room.  She has a Back To School SLP Checklist to make sure you have everything you need at the start of the year.  I really her Norm Referenced Assessment Quick Look Sheet and her Language Sample Checklist for assessments.  They are easy to use and give great information about a student!
